This form should be used by a Council’s Health Services Chairman, their appointee, or the person within your Council responsible for reporting numbers such as these.

Uses of this form:
* A council organized a blood drive and solicited donations from their parish and/or the community.
* A council’s members donated blood independently of a blood drive organized by the council.

Once reported, your data will be part of the Alabama State Council’s blood drive statistics.
